Individual Permits

All new and expanding facilities MUST submit an Engineering Alternatives Analysis (EAA) in addition to any required application(s).

Major Municipal: Any permit with a permitted flow of 1.0 MGD or higher.

Major Non-Municipal: Any permit with a score of 80 or higher on the EPA Ratings Sheet.

Municipal POTW and 100% Domestic Facilities - Application Check List

• EPA Form 2A: Municipal w/Design Flow >0.1 MGD -OR- having pretreatment programs
• EPA Form 2A Instructions
• NC Form A: Municipal w/Design Flow <0.1 MGD and NO Pretreatment
• NC Form D: Privately Owned, 100% Domestic Facilities w/Design Flow <1.0 MGD

Industrial, Manufacturing & Commercial Facilities

If you are not sure whether your facility is a major or minor, please contact the NPDES Unit.

MAJOR INDUSTRIAL FACILITIES

• EPA Form 1: Major Industrial Facilities, plus 1 of the following:

EPA Form 2C: Facilities that discharge process wastewater
- OR -
EPA Form 2E: Facilities that do not discharge process wastewater

• EPA Form 2F: Major Industrial facilities with stormwater outfalls MUST fill this out for each stormwater-out outall covered by NPDES permit

MINOR INDUSTRIAL FACILITIES

• NC Form C-Seafood: Seafood Processors
• NC Form C-MI: All other minor industrials
   

Other Facilities

• NC Form C-WTP: Water Treatment Plant (All Types)
• NC Form C-GW: Groundwater Remediation (For Solvents)

Note: Refer to general permits for petroleum based groundwater remediation.
