Clean Water Needs Survey
The 2012 Survey is Underway!
The Clean Water Needs Survey is conducted every four years as required by the Clean Water Act. The results of the survey may determine how State Revolving Funds will be allotted to North Carolina in the future so your responses are important.
If your Community’s Population is less than 10,000 the only documentation needed is this FORM completed and signed by a local official representing the community (Signature Box #2, Page 6).
If your Community’s Population is larger than 10,000 please provide us with supporting documentation (CIPs, Engineering Reports, Unfunded applications, etc.) justifying the needs and the cost. A List of acceptable documents is given in Appendix 1 (Page 7).
