OSHA Required Recordkeeping
The North Carolina Department of Labor, Division of Occupational Safety and Health requires employers to compile certain injury and illness information and then report that information each year to their employees. The report formats used to collect and display the information are called the OSHA Form 300 Log of Work-Related Injuries and Illnesses (Log), and the OSHA Form 300A Summary of Work-Related Injuries and Illnesses (Summary). The Log collects the injury and illness data whereas the Summary displays the data. In the summary form, the injury and illness data that could be used to identify a particular person is removed.
The report forms are available in the below links as either a pdf file or an Excel spreadseet. If you download the pdf file, you will need to print then fill in the paper form. If you download the Excel spreadsheet, you can enter the requested information directly into the Log portion of the spreadsheet, and the Summary information will be automatically calculated for you.

The is Log of Work-Related Injuries and Illnesses is used to classify work-related, recordable injuries and illnesses and to note the extent and severity of each case. When an incident occurs, use the to Log to record specific details about what happened and how it happened. Click here to see how to record an injury or illness.
The Summary of Work-Related Injuries and Illnesses — a separate form — shows the work-related injury and illness totals for the year in each category. At the end of the year, count the number of incidents in each category and transfer the totals from the Log to the Summary. If a location did not have any recordable injuries or illnesses for the year in question, you still must fill out the Log and Summary, however, place a zero in each of the Summary form boxes (G through M).
Then post the in a visible location so that your employees are aware of injuries and illnesses occurring in their workplace. The Summary is to be posted from February 1st through April 30th of each year. After the required display period has been met, the Summary may be removed, however both the Log and Summary for a particular year are to be maintained for five (5) years.
If your Division, Office or Program has more than one physical location (refered to as a site or establishment), you must prepare a separate records for each physical location that is expected to remain in operation for one year or longer.
